I normally don't like to blog about work per se, but I just can't help bitching tonight. Maybe it's the check-up tomorrow getting to me. Had a talk with one of my registrars over dinner today, he reckons my Wenckebach must surely count for something. We'll see.

In any case, I doubled our ICU's occupancy tonight. And there's an arterial sheath to be removed at 1130pm (or, God forbid, 230am ...). And a massive pulmonary embolism came in just now. Although I've intubated quite a few patients (not something to be happy about), this is my first real rapid sequence intubation - intubating an awake patient. The last RSI was one that I assisted with giving the suxamethonium, in NUH, with a very very nice Reg. It went quite alright, although he is a huge man, and initially I got a little worried since his saturation wasn't picking up after intubation - then I realise that he's probably having quite a large ventilation-perfusion mismatch due to the PE. Oh well. Thrombolysed him and he collapsed 3 times in front of me, twice with PEA and one Torsades. Welcome to cardiology.

GREAT. As I'm typing this, I hear the nurses saying, "Is he waking up? Yes, I think he's waking up." Not good. I shall sedate him again.
